Summary
This study is testing a 6-month digital support program designed to help people with heart failure better manage their condition. Researchers want to see if using this program improves patients' sense of control over their health, their quality of life, and their self-care habits. The study will also compare two different ways of introducing the program to clinics to see which works best.
