Summary
This study tests a new way to care for heart failure patients in Uganda using a smartphone app and a standard treatment bundle. About 576 adults with moderate heart failure will use the app to report symptoms and get guidance, while doctors monitor them remotely. The goal is to improve self-care and reduce deaths or hospital stays.
