Summary

This study tests a 'Digital Bridge' that connects communication tools already used by hospitals and family doctors to improve care for older adults with multiple chronic conditions. The goal is to make hospital-to-home transitions smoother and reduce the risk of readmission. Researchers will work with patients, caregivers, and healthcare providers to design and test the system in two hospital networks in Ontario, Canada, involving 640 participants.
