Digital bridge aims to smooth Hospital-to-Home transition for seniors

Summary
This study tests a 'Digital Bridge' that connects hospital and primary care communication tools to improve care transitions for older adults with multiple chronic conditions. Researchers will work with patients, families, and providers to design and adopt the technology in two hospital systems. The goal is to see if better communication reduces readmissions and improves quality of life. 640 participants aged 60 and older will be followed from admission to 6 months after discharge.
