Summary
This study tests whether using a smartphone app for remote health management can reduce the risk of heart disease better than regular doctor visits. About 790 adults with high heart disease risk will be split into two groups: one using the app and one getting usual care. The goal is to see if the app helps lower blood pressure, blood sugar, cholesterol, and improve medication use and lifestyle.
