Summary
This study tests whether using a smartphone app for remote health management can lower the risk of heart disease better than standard doctor visits. About 790 adults with a high 10-year risk of heart problems will be split into two groups: one using the app and one getting usual care. The goal is to see if the app helps improve blood pressure, blood sugar, cholesterol, and lifestyle habits.
