Summary
This study is creating a patient registry to track the natural progression of diabetic kidney disease in real-world settings, focusing on underserved communities. It will follow 2000 adults with type 2 diabetes and kidney disease for up to two years, collecting health information from medical records and questionnaires. The goal is to understand how the disease progresses, what treatments patients receive, and what factors influence outcomes, without providing any experimental treatments.
