Summary
This study follows 2,000 adults with type 2 diabetes and kidney disease from underserved communities for up to 2 years. Researchers will collect health data from medical records and surveys to see how the disease progresses and how it is treated in real-world settings. No new treatments are tested—the goal is to gather information to guide future research and improve care.
