Summary
This study looks at whether a pharmacist-led review can safely reduce the number of medications taken by adults aged 75 and older who currently take six or more drugs. The goal is to see if cutting back leads to fewer falls, fewer hospital stays, and a better quality of life. Researchers will track changes in medication use and how well the remaining drugs fit each person's needs.
