Summary
This study tested whether a special 'return home' consultation, involving the patient, their regular doctor, and a cancer specialist, could reduce unplanned hospital visits for people with metastatic cancer. 667 hospitalized patients were randomly assigned to either this organized consultation or standard discharge care. The goal was to see if better coordination at discharge leads to fewer emergency visits and readmissions.
