This study looked at whether community health workers (CHWs) can improve health outcomes for people with chronic conditions like diabetes, high blood pressure, and obesity. Over 10 months, 202 low-income adults from specific zip codes received either CHW support plus standard care, or just standard care. The goal was to see if CHW navigation reduces stress and improves health markers such as blood sugar, cholesterol, and blood pressure.
