Summary
This study tested whether a community health worker (CHW) program is practical and well-liked by caregivers of people with dementia after a hospital stay. Researchers enrolled 34 patient-caregiver pairs and provided telephone coaching sessions. The goal was to see if caregivers found the support helpful and if the program could be carried out as planned.
