New nursing approach aims to empower chronic disease patients

Summary
This study tests whether a special nursing care model helps people with long-term health conditions like diabetes or heart disease work better with their nurse to manage their own health. About 420 adults who already see an advanced practice nurse will take part. The goal is to see if this approach improves patients' ability to self-manage their condition.
