Summary
This study looks at whether a special nursing care model helps people with long-term health conditions like diabetes, heart disease, or Parkinson's work better with their advanced practice nurse. About 420 adults will be followed for 9 months to see if they feel more in control of their own health. The goal is to improve how patients and nurses partner together for better daily management.
