Summary

This study compares a continuous glucose monitor (CGM) to standard fingerstick checks in 160 adults with type 2 diabetes who use insulin after leaving the hospital. Participants will use either a CGM or fingerstick testing for 12 weeks to see which method better controls blood sugar. The goal is to improve diabetes management during this critical transition period.
