Summary
This study looks at whether wearing a continuous glucose monitor (CGM) after leaving the hospital helps people with type 2 diabetes control their blood sugar. About 108 adults who need insulin will wear a CGM for 12 weeks and get personalized alerts. The goal is to see if this tool improves time in a healthy blood sugar range and patient satisfaction.
