Summary
This study aims to understand how a structured approach to managing heart health risks works in everyday doctor's offices. Researchers will observe 220 adults who already have risk factors like high blood pressure or diabetes, but will not change their current treatments. The goal is to collect information to help doctors make better decisions about preventing heart problems in the future.
