Summary
This study observes 50 young adults with chronic inflammatory rheumatic diseases to see which transition program works best when moving from pediatric to adult care. Some participants received a special education workshop plus a joint doctor visit, while others had only the joint visit. The goal is to find out which approach helps them manage their own health better after the switch.
