Summary
This small pilot study is testing a new program to help young adults (ages 18-35) recently diagnosed with bipolar disorder transition from hospital to community care. The program includes group education, one-on-one peer support, and help for community healthcare providers. The goal is to see if the program is feasible and acceptable, and to gather early data on its potential to improve symptoms and treatment adherence.
