Summary

This study tested whether adding a pharmacist to a hospital's mobile geriatric team could help improve medication safety for elderly patients. The team reviewed the medications of 200 patients aged 75 and older who were taking five or more daily drugs. The goal was to identify and reduce potentially harmful or unnecessary prescriptions during and after their hospital stay.
