Summary
This study tested whether adding a pharmacist to a hospital's mobile geriatric team could help improve medication safety for elderly patients. The team reviewed the drug lists of patients aged 75+ who were taking five or more daily medications. The goal was to identify and reduce potentially unsafe prescriptions during and after their hospital stay.
