Summary
This study tested strategies to help community health centers routinely ask patients with or at risk for diabetes about social needs like food, housing, and transportation. The goal was to see if using electronic health record tools could increase how often these needs are documented and addressed. 31 participants were involved, and the study compared screening rates before, during, and after the intervention.
