Summary
This study is testing whether a personalized smartphone app can help people with Type 2 Diabetes better manage their condition. Researchers will compare the app, which tracks health data and connects patients with their care team, against standard care. The main goal is to see if using the app leads to improved long-term blood sugar levels over 18 months.
