Summary
This study is testing whether a smartphone app called a 'Smart Family Doctor,' which uses artificial intelligence, can help people better manage their health after heart procedures. It will involve over 500 adults who have had heart surgery or stents and also have high blood pressure, diabetes, or high cholesterol. The goal is to see if the app helps them improve control of these conditions and their overall quality of life.
