Summary
This study tests whether an AI chatbot, combined with nurse support, can help 72 adults with early-stage cardiovascular-kidney-metabolic (CKM) syndrome better manage their symptoms and reduce hospital visits. Participants will use a smartphone app to report symptoms weekly and receive personalized feedback. The goal is to see if this approach improves symptom burden, patient confidence, and health literacy compared to standard monitoring alone.
