Summary
This study looks at whether group visits in primary care can help adults aged 70 and older complete advance care planning documents, like living wills. About 516 participants will either join two 2-hour group discussions or receive mailed resources. The goal is to see if the group approach leads to more people having their healthcare wishes on file.
