Summary

This study looks at ways to help teenagers and young adults with congenital heart disease (CHD) switch from pediatric to adult doctors. Researchers will compare education led by nurses versus doctors to see which works better. About 450 people with CHD, their supporters, and providers will take part. The goal is to improve tools and make the transition smoother.
