Summary
This study will test a new program where Indigenous patient navigators help young adults (ages 16-25) with chronic health conditions as they move from pediatric to adult healthcare. The program was co-designed with Elders, community members, and healthcare providers in Maskwacis, Alberta. About 20 participants will receive support and share their experiences through interviews and talking circles to see what works and what can be improved.
