Summary
This study tested whether a special web-based program could help patients with heart failure, their family caregivers, and their nurses communicate better. The goal was to see if better communication could improve patients' quality of life and make care more focused on the patient's needs. About 150 people participated, including patients, their main family caregiver, and experienced cardiac nurses.
