Summary

This pilot study tests a simple daily weight tracking tool called 'Wake and Weigh' to see if it helps older adults (55+) with heart failure improve their self-care and quality of life. Participants will use the tool in the hospital and at home for four weeks. The study aims to see if this approach is practical and effective before testing it in a larger trial.
