Hospital at home: study sends patients to recover in their own beds

Summary
This study is testing if it's practical for certain hospitalized patients to be sent home early and continue their care through a 'Virtual Ward.' Patients would be monitored remotely from home using a smartphone app to track their vital signs and symptoms. The main goal is to see if at least 30% of eligible patients can safely make this switch from the hospital to their own home.
