Summary
This study is testing a program called MINDSET for veterans with heart disease or diabetes who continue to smoke or drink heavily despite previous advice from their doctor. The program involves sending a personalized information packet by mail followed by a supportive phone call, aiming to help veterans feel more ready and able to change their habits. Researchers will compare this approach to usual care to see if it better helps veterans reduce their tobacco and alcohol use over one year.
