Summary
This VA study is testing two new support programs to help primary care teams better manage Veterans with multiple chronic health conditions who are at high risk of being hospitalized. The programs involve more thorough health assessments and phone-based coaching to help patients stick to their medications. The goal is to find the best way to deliver this proven care, improve Veterans' health, and reduce avoidable hospital visits.
