VA aims to stop Veterans' Post-Hospital decline with new recovery program

Summary
This study is developing and testing a new phone-based support program for older veterans who have just left the hospital. The goal is to help them regain their ability to do daily activities like dressing, bathing, and moving around at home. Researchers will work with 40 veterans to see if the program is practical and helpful for preventing loss of independence after a hospital stay.
