First seen Jun 27, 2026 · Last updated Jul 17, 2026 · Updated 2 times
Summary
This study looks at a program called Proactive Care, which gives personalized support to people with chronic diseases. The goal is to see if this help can reduce how often people need to go to the hospital. About 5,000 adults with high expected medical costs will be randomly chosen to either get the program right away or join a waitlist. Researchers will then compare how many days each group spends alive and out of the hospital.
