Summary
This study aims to better identify when a patient's trip to the emergency room is caused by a harmful reaction to their medications. Researchers will use a special checklist on 100 older adult patients to spot these medication-related problems. The goal is to understand how often this happens so hospitals can improve medication safety and prevent future harm.
