Summary
This study is testing a new way to deliver palliative care at home for people with advanced heart failure or certain cancers. The care team grows as a patient's symptoms get worse, adding more specialists like nurses or doctors when needed. The goal is to see if this flexible approach helps manage symptoms better, improves quality of life, and reduces trips to the hospital.
