Summary
This study tested a new system to help community health centers ask patients about life challenges—like housing, food, or transportation—that can affect diabetes. Researchers worked with 30 clinics to see if training them to collect this information and connect patients to helpful resources would improve diabetes care. They measured if this approach led to better patient health, like improved blood sugar and blood pressure control.
