Summary

This study tests whether a phone-based coaching program (TeleSCoP) can help people who had an ischemic stroke feel more confident managing their health, lower risk factors like blood pressure and cholesterol, and reduce repeat hospital visits. Sixty participants will receive either standard care or extra coaching sessions and educational videos over three months. The goal is to see if remote support can improve recovery and prevent future problems.
