Digital heart helper: swiss trial tests telemedicine for better heart failure care

Summary

This study tests whether using telemedicine (smartphone or tablet check-ins) can help doctors fine-tune heart failure medications better than standard care. About 140 adults hospitalized with heart failure will be followed for 3 months after discharge. The goal is to see if digital monitoring leads to safer and more effective drug adjustments.
