Summary
This study tests a telehealth training program for primary care teams in New Mexico. The program connects local clinic staff with specialist mentors to improve care for patients with diabetes and other ongoing health issues like high blood pressure or depression. Researchers will compare patient blood sugar levels from clinics using this training to those using standard care to see if the remote mentoring helps.
