Phone calls and home checks: a new plan to tame high blood pressure after the ER

NCT ID NCT06247111

Summary

This small pilot study is testing if a simple telehealth program can help people better manage their high blood pressure after a trip to the emergency room. The program involves checking blood pressure at home and having weekly phone check-ins with a pharmacist to adjust medications if needed. Researchers want to see if this approach is practical and helpful for the 24 participants over a 3-month period.
