Summary

This study is testing whether a culturally tailored telehealth program, where community health workers coach patients remotely, can help South Asian immigrants better control their diabetes and high blood pressure. It will compare this coaching approach to usual medical care over 12 months. The goal is to see if this model can be effectively adopted and maintained in real-world community and clinic settings.
