Summary
This study is testing whether a virtual visit with a pharmacist after a hospital stay can help people with COPD. The goal is to teach patients how to use their inhalers correctly and manage their medications at home, which may help them avoid needing to return to the emergency room or be readmitted to the hospital. Researchers will compare outcomes for patients who receive these telehealth visits against those who get usual care.
