Summary
This study tests whether follow-up care for stroke survivors can be done effectively through video or phone calls with a nurse, instead of in-person clinic visits. About 196 adults who recently had a stroke and own a smartphone will take part. The goal is to see if telecare improves recovery, quality of life, and reduces the chance of another stroke.
