Summary
This study is testing whether follow-up care for stroke survivors can be done effectively through video calls with specialized nurses instead of requiring in-person clinic visits. Researchers will enroll 196 recent stroke patients who are going home and have a smartphone. They will compare health outcomes like disability levels, quality of life, and hospital visits between patients receiving standard care and those receiving the telecare model.
