Summary
This study is testing if using technology can help people with type 2 diabetes who are starting insulin during a hospital stay manage their condition better when they go home. It will compare a group using a continuous glucose monitor and a smart insulin pen cap that provides reminders and data to a group using standard care. The goal is to see if the technology helps patients take their insulin on time and improves their blood sugar control over 12 weeks after leaving the hospital.
