Summary
This study is testing a new team-based program to help people better manage their high blood pressure. It will involve 200 African American and Latinx adults from Chicago's West Side who have uncontrolled hypertension. The program provides remote blood pressure monitoring, support from a nurse, pharmacist, and social worker, and a mobile app for education, and will be compared against standard care with educational materials.
