Summary
This study looks at how to help primary care clinics set up a program where patients with high blood pressure send their home readings to their doctor's office. The goal is to see which strategies work best to improve blood pressure control and reduce differences in care between Black and White patients. About 5,760 patients and their clinic staff will take part, and researchers will track changes in blood pressure over time.
