Hospital teams aim to keep HIV patients connected to care

Summary
This study is testing whether a team-based support program in the hospital helps people with HIV get connected to ongoing specialist care after they go home. It will enroll 250 adults who are hospitalized and not currently receiving proper HIV treatment. The main goal is to see if this support leads to more patients seeing an HIV doctor after discharge and reduces how often they need to return to the hospital.
